How to read these numbers

Number of Deficiencies Found denotes the number of significant and minor deficiencies found during a sanitary survey/site visit.

Number of Deficiencies Resolved indicates how many of the significant and minor deficiencies identified during the sanitary survey/site visit have been resolved.

Both explanations come from the help text published with these pages.

3 of the visits above record a recommendation and a count of zero. That is not a contradiction. The help text published with these pages defines that code: A recommendation is not considered a deficiency[.] So a recommendation is reported as the highest severity without being counted in either column.

One visit above records a significant or minor severity while its own count is zero. The severity is the highest deficiency identified during the visit including any associated from earlier site visits, which the counts do not include. The two columns do not describe the same span of time.

What a site visit is

Representatives of regulating agencies conduct site visits. A regulating agency is a government agency that has rule enforcement authority over assigned public water systems. Possible reasons for conducting a site visit include: performing a compliance inspection, investigating a complaint, providing technical support (e.g., training), and inspecting construction. One of the most common reasons for a site visit is to perform a sanitary survey.
